Denver Endocrinology, Diabetes and Thyroid Center, PC’s goal is to provide quality service to all of our patients in a timely manner. Failure to keep scheduled appointments is costly to both the clinic and our patients. This notice explains our policy regarding No Shows and Late Cancellations.
Patients who are unable to keep their appointments are requested to provide at least 24 hours’ notice prior to the scheduled appointment time. We understand that emergencies may occur and will consider individual circumstances on a case-by-case basis. Providing adequate notice allows the clinic to offer appointment times to other patients and use provider time efficiently.
A No Show is defined as failing to appear for a scheduled appointment without prior notice.
A Late Cancellation is defined as canceling an appointment with less than 24 hours’ notice.
For established patients, failure to provide the required notice for a No Show or Late Cancellation will result in a $50.00 fee, which will be billed to the patient’s account. Patients who arrive late to their scheduled appointment time and cannot be seen as a result, requiring the appointment to be rescheduled, will also be assessed a $50.00 late arrival fee. Additionally, if a patient confirms an appointment and then fails to keep that appointment, a $50.00 fee will be billed.
Patients who miss appointments on a regular basis, or who have three (3) consecutive No Shows, Late Cancellations, or Late Arrivals requiring rescheduling, may be considered for dismissal from the practice.
I have read and understand this No Show and Late Cancellation Policy and accept responsibility for its terms.
